Office Information: About Your Visit
Insurance Information
We are a provider for most insurance networks, including Medicare. Our business office will file claims under any insurance plan that you may have, but you might be subject to out of network costs if we are not a “network provider.” For the most up-to-date information and explanation of benefits under your plan, please contact our billing department and ask for the insurance verification specialist.
Appointments
Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
Fees & Payments
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We will accept cash, check, money order, and all major credit and debit cards for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.
Prescriptions & Renewals
We do not prescribe new medications without examining you. Refills of certain medications will be handled over the phone only during business hours and if you are under continuing care. All written prescriptions for medications will be picked up from our office by the patient or other authorized person. Prescriptions will not be sent through the mail.
Do not wait until you are out of medicine to call. It normally takes 48 business hours for your request to be processed, as the volume of the request exceeds our daily capacity to fill them.
Emergencies
Our emergency number is
(850) 863-2153. It is answered 24 hours a day either by the office staff or the answering service. They will give a message to your doctor or the doctor "on call." We will make every effort to care for your problem promptly either in the office or at the Emergency Room. If you are treated at the Emergency Room at Fort Walton Beach Medical Center, Twin Cities Hospital, Eglin Hospital, or Sacred Heart Hospital Emergency Care, please request that we be called.
